# The Programme

**One-to-one specialist exercise rehabilitation for men with prostate cancer and survivors.**

If prostate cancer treatment has left you tired, weaker, or unsure how to move, you're not imagining it. The Programme is one-to-one specialist exercise rehabilitation, built around you. You work directly with Piotr Kardas, week by week, at a pace your body sets.

Your sessions are led by Piotr Kardas, a Prostate Cancer Exercise Specialist, Registered Clinical Exercise Physiologist. Piotr delivers specialist exercise rehabilitation for men with prostate cancer, at a world-leading NHS cancer centre, as a clinical exercise physiologist.

> Always consult your oncologist or consultant before beginning any exercise programme.

## What does a prostate cancer exercise programme involve?

A prostate cancer exercise programme is structured, supervised exercise designed around your treatment and your fitness. NICE NG131 recommends supervised resistance and aerobic exercise for people starting or having hormone therapy. You work one-to-one with a specialist who assesses you, sets a plan, and adjusts it as you progress.

Nothing is off-the-shelf. Your plan reflects where you are in treatment, how you feel, and what you want to be able to do. Supervised resistance and aerobic exercise has been shown to reduce fatigue and improve quality of life (NICE NG131). The work is steady, individual, and reviewed as you go.

## Who this is for

This programme is for men living with prostate cancer, and for survivors. It suits men who are starting or having hormone therapy, recovering from surgery, or moving through other treatment. It works across a wide range of fitness levels.

You do not need to be active already, or any special equipment to begin. If you're unsure whether this fits you, the consultation is the place to ask.

## How the programme works

Three steps shape it. It starts with an assessment: how you move, how you feel, where you are in treatment. From that, an individual plan. Then resistance and aerobic exercise, progressed over time. Your plan changes as you change, so the work always fits you.

Resistance and aerobic exercise are the main work, side by side. You work on strength with resistance or weights, matched to your level. Aerobic fitness comes in too, at an effort that suits you. As the work progresses, the load and pace move up gradually, and never faster than your body allows.

## How many supervised exercise sessions does NICE recommend?

NICE NG131 recommends supervised resistance and aerobic exercise at least twice a week for 12 weeks, for people starting or having hormone therapy. Twice a week across 12 weeks works out to a minimum of 24 supervised sessions. NICE links this to reduced fatigue and improved quality of life.

The guideline puts it this way:

> "Offer people who are starting or having androgen deprivation therapy supervised resistance and aerobic exercise at least twice a week for 12 weeks to reduce fatigue and improve quality of life."
>
> NICE NG131, recommendation 1.4.19. Source: https://www.nice.org.uk/guidance/ng131

The 24-session figure is simple arithmetic from the guideline, not a package we sell. How often you see Piotr is agreed with you at your consultation, and he can advise on exercise between appointments.

## The methods your programme can draw on

Your programme can bring in several movement methods, chosen for you. These include Tai Chi, Qigong, Pilates, and progressive resistance or weight training. They are adapted methods Piotr draws on, not separate products you buy.

Resistance and weight training is the exercise NICE NG131 names, alongside aerobic work, for people on hormone therapy. The gentler methods work on balance, breathing, and controlled movement early on. Over time, your plan builds toward the resistance and aerobic work.

## How your sessions are delivered

Sessions run in two ways. Online by video call, anywhere in the UK. Or as a home visit in London or Surrey, arranged with Piotr and priced for your circumstances.

Both are one-to-one, and both follow the same individual plan, so you can move between the two as suits you.

Sessions are usually offered on Monday, Wednesday and Thursday evenings at 6pm, 7pm and 8pm, and on Saturdays hourly from 9am to 2pm. Consultations are usually offered on Tuesday evenings, at 6pm, 6.30pm, 7pm, 7.30pm and 8pm. These are the times Piotr usually opens, not a fixed timetable. Appointment times are agreed with Piotr when he arranges them, and other times can be arranged with him.

## What happens in a session?

An online appointment runs for 55 minutes and a home visit for around an hour, always one to one. You start with a check-in: how you feel, how you recovered from last time. Then the planned resistance and aerobic exercise, at a level matched to you. A cool-down closes it, with a note on what to adjust next.

Every session is supervised in real time. Piotr watches your technique, corrects it, and sets the right load for the day. Nothing is rushed, and you set the pace with him.

## Your options

One consultation first, then what suits you. Everything starts with the paid 15-minute consultation. If you decide to go on, Piotr talks you through the options that fit your situation, then sends you a secure payment link. There is no package to commit to, no subscription and no automatic renewal.

Online appointment. From £135. A single 55-minute one-to-one session with Piotr by video call, anywhere in the United Kingdom.

Home visit. Priced for you. A single appointment at your home in London or Surrey. The price depends on where you live and what you need.

Exercise plan. From £65. Prepared for you by Piotr, in an app on your phone at no extra cost, or on paper.

Exercise toolbox. £45, £95 or £145. A small, medium or large set of exercise equipment, put together by Piotr and adjusted to you, plus £4.99 postage and packing.

Piotr puts each toolbox together himself and adjusts it to what you need. He has the same equipment on his side of the screen, so he can show you each exercise exactly. He chooses every piece himself, so he knows its quality.

NICE NG131 recommends supervised exercise at least twice a week for 12 weeks for people on hormone therapy. Piotr talks with you about how often to meet and what to do between appointments. The terms of purchase are set out in the Service Terms.

Purchases are covered by our [service terms](/legal/service-terms) and your [right to cancel](/legal/cooling-off-and-cancellation).

## Is supervised exercise better than exercising alone?

NICE NG131 recommends supervised resistance and aerobic exercise. Supervision is written into the guideline itself. With a specialist present, your programme is designed for you, watched closely, and adjusted as you change. The load and technique are matched to you.

Supervision also means Piotr watches how you respond as you go. He spots what needs to change, and keeps the exercises appropriate to your treatment and paced to you. Always speak with your oncologist or consultant before you begin.

## Can I take part during hormone therapy?

NICE NG131 addresses exercise for people starting or having hormone therapy, also called androgen deprivation therapy. The programme is designed with hormone therapy in mind, and your plan is adapted to your treatment.

Speak with your oncologist or consultant first, and share any advice they give you. That advice shapes how your programme is built.

## Do I need to be fit before I begin?

No. The programme starts from where you are now. Your first session is an assessment, and everything is built around it. You progress at a pace matched to your body and your treatment.

There is no minimum level of fitness to start. Always consult your oncologist or consultant before beginning any exercise programme.

## How do I start?

You start by booking a paid 15-minute online video consultation. On the call, you talk through your treatment, how you feel, and what you want to work on. Piotr explains how the programme would fit you. If you decide to go on, he suggests what suits you, such as single online appointments, a home visit, an exercise plan or an exercise toolbox, and sends a secure payment link. There is no package.

There is no obligation to continue after the consultation. It is a short, honest conversation about whether this is right for you.

## Book your 15-minute consultation

Every programme starts with one paid conversation. The consultation lasts 15 minutes and runs online, by video call. If you decide to go on, Piotr suggests what suits you and arranges it with you directly.

On the call, you will:

- talk through where you are in treatment and how you feel
- say what you want to be able to do again
- hear how Piotr would shape the programme around you
- ask anything you like, with no obligation to continue
